All that is required is to use the Order Form or email link below and email or fax the following information:

Your name, billing and shipping address, phone number, email address, the artist and title of the album or song,

We also need to know which design that you wish to purchase.

Custom designs may not list prices because price depends on quantity and other variables.
Please do not hesitate to contact us for price quotes on any item and mention the design #

All RIAA Certified Awards must list your detailed album credits in order for us to obtain permission to make your plaque(s)
